A Thread of Hope for the Future

In healthcare, we often talk about “upstream” and “downstream” thinking. Upstream thinking focuses on health promotion and disease prevention, taking proactive steps to reduce the risk of illness before it begins. Downstream thinking, on the other hand, is reactive; it addresses illness after it has already developed. Unfortunately, I believe that many of us live downstream; we tend not to think about our health until something goes wrong.
Stats Canada gives evidence to this with the report of rising rates of chronic conditions such as heart disease, respiratory illness, diabetes, and increasing mental health needs. As a nurse, I find myself asking: how can we prevent or at least reduce progressive disease/illness? Especially with the current state of healthcare and the current state of the world in which we live. It is hard to have a positive outlook for the future.
Recently, my grandson and I toured the United Nations in New York City. Our guide offered an engaging and informative overview of the UN’s councils and key agencies. He explained how each contributes to the UN’s mission: maintaining international peace and security, fostering cooperation among nations, protecting human rights, and striving toward a world of peace, dignity, equality, and a healthy planet.
The experience was not only educational, but it was also deeply inspiring. Hearing about the collective efforts of millions working toward a healthier world left a strong impression on me. But what moved me most was watching my 9-year-old grandson. He was fully engaged, listening closely, asking thoughtful questions, and genuinely curious about how he could help make the world better: upstream thinking. In that moment, I saw a child who was unjaded, open-hearted, and eager to learn. Beyond feeling like a proud grandmother, I felt something even greater: hope for the future.
Hope is a feeling of expectation or desire for a certain thing to happen. It is proactive, and in a way, upstream thinking. Even when we cling on to the thinnest thread of hope, we create resilience to hang on and sustain the challenges because we believe and/or desire something better, something brighter in our future.
